8/21/2023 EnchantaVaughn Approved (AM) QMHP-C welcomed client back to the school setting. Client was observed entering class in a responsible manner, as evidenced by him saying good morning and turning in his homework. As reported, client was attentive and engaging in class. Client expressed great verbal and nonverbal communication and was supported for his cooperation toward peers and authority. Client was able to complete his work then take a small break. Client got a drink of water and walked with QMHP to the break room. Client held a conversation about all the things he looked forward to this week. Client was encouraged to go back to class and participate with his classmates so that he can understand the new curriculum. Client was supported for his patience and interacting well with others and using his independent time to catch up on old work. Client has shown improvement in structured setting and following authority directives the first time they are given. Client was able to participate in team building activities with no need for redirection or interaction with QMHP regarding behavioral adjustments. Client was observed displaying positive behaviors, thinking and exercising his coping skills by QMHP. Client walked with his counselor to attend group therapy. Client communicated that he did not get into any trouble today and was happy about it. Client was given praise for doing a good job. Client displays respectful manners in the hallway. Client enjoys being around his group members, as evidenced by him laughing and smiling. Client seem to be intrigued by the interventions and learning about his mental health. Client was witnessed sitting next to his peer making appropriate comments about their day. Client remained in a cheerful mood. Client transitioned back into the academic setting displaying polite manners. Client was observed displaying positive behavior by displaying on task behavior by completing class assignments without being distracted. As time progressed, the client has demonstrated a cooperative attitude while being supportive of others. Client followed his instructions and stayed on task until dismissal.
